Step 2

1. The user's menu page displays the purpose(s) for which the user may access DMV records
2. Select "Insurance Underwriting" to access driver records for insurance underwriting purposes.
3. Select "Customer Utilities" to change your user password or to view a log of DMV records that you have accessed.

Step 3

1. You may search for driver records by entering the driver number or the driver's name, birth date, and sex.
2. A driver transcript report is the default report selection. Alternatively, you may request other options.
3. The "User Comments Option" permits you to enter a customer name, policy number, or other relevant information that will appear at the top of the transcript for your reference.

Step 4

1. The system will display the record that you requested, exactly as it is formatted from the DMV database.
2. Using your Web browser, you may print a copy of the record.
