License & Case Decision Notification Service Addendum

This addendum to the License & Case Decision Notification Service outlines the terms and conditions/stipulations surrounding the use of the data provided through this service. This service was developed by the Virginia Interactive (VI) in partnership with the Virginia Department of Health Professions (DHP). The service provides entities the ability to track the status of licensed health professionals in the Commonwealth by sending an alert when there has been a change in the status of an individual?s license or case decision information that is being tracked. The alert is sent to an email address that may go to a PC, pager, cell phone, or any other handheld device that receives email. Alerts are sent daily, Sunday through Friday.

Individuals using the License & Case Decision Notification Service must agree to the following:
1.The data is used solely for informational purposes.
2.The user is responsible for payment of the full amount of the annual fee within thirty days of signing up for the service. This annual fee includes the VI Premium Subscriber fee.
3.The annual fee will be based the notification access level selected.
4.The data is not to be resold for revenue or profit without the signed, written consent of the Virginia Department of Health Professionals.
